Roles & Responsibilities
- Provide compassionate direct nursing care to residents, focusing on dignity and comfort
- Administer medications and treatments accurately and document in the chart
- Assess resident condition and communicate changes promptly to the care team
- Collaborate with nurses, therapists, and providers to develop and follow care plans
- Support admissions, transfers, and discharge processes with thorough documentation
- Educate residents and families about care plans, medications, and safety measures
Qualifications
- Current and active LPN license (Massachusetts preferred or multistate as applicable)
- Completion of an accredited LPN/LVN program
- Current BLS/CPR certification (or willingness to obtain upon hire)
- Prior experience in long-term care, skilled nursing, or rehab settings is a plus
- Strong communication, documentation, and teamwork skills
- Ability to pass facility background checks and meet health screening requirements
Education
- Graduate of an accredited Licensed Practical Nurse program
- High school diploma or equivalent required
Knowledge & Skills
- Medication administration and basic pharmacology knowledge
- Experience with wound care, ADLs assistance, and chronic condition management
- Familiarity with electronic medical records and accurate charting practices
- Ability to perform assessments, take vital signs, and recognize clinical changes
- Good time management and prioritization in a fast-paced rehab environment
- Strong interpersonal skills to work effectively with residents and multidisciplinary teams
