Roles & Responsibilities
- Provide compassionate hands-on nursing care to residents in a rehab and long-term care setting
- Administer medications and treatments accurately and safely
- Perform wound care, vital sign monitoring, and ADL assistance with dignity
- Document resident status, care provided, and changes accurately in the chart/EMR
- Collaborate with RNs, therapists, and multidisciplinary team members to implement care plans
- Communicate clearly and empathetically with residents and families about care and progress
- Support a positive safety culture by following policies, reporting concerns, and participating in quality initiatives
Qualifications
- Current LPN license (Licensed Practical Nurse) in Massachusetts or eligible for state licensure
- Completion of an accredited practical nursing program
- Basic Life Support (BLS) / CPR certification (or willing to obtain)
- Comfortable administering meds, IV/PO documentation, and basic nursing procedures
- Excellent communication skills and a team-first attitude
- Previous experience in rehab, skilled nursing, or long-term care preferred
Education
- Graduate of an accredited Practical Nursing program (LPN)
Knowledge & Skills
- Medication administration and safe documentation practices
- Wound care basics and pressure injury prevention
- Ability to take and interpret vital signs and report changes
- Familiarity with electronic medical records and point-of-care charting
- Strong resident-centered care approach and empathy
- Time management and prioritization during busy shifts
