Roles & Responsibilities

• Provide compassionate hands-on nursing care to residents in a rehab and long-term care setting
• Administer medications and treatments accurately and safely
• Perform wound care, vital sign monitoring, and ADL assistance with dignity
• Document resident status, care provided, and changes accurately in the chart/EMR
• Collaborate with RNs, therapists, and multidisciplinary team members to implement care plans
• Communicate clearly and empathetically with residents and families about care and progress
• Support a positive safety culture by following policies, reporting concerns, and participating in quality initiatives

Qualifications

• Current LPN license (Licensed Practical Nurse) in Massachusetts or eligible for state licensure
• Completion of an accredited practical nursing program
• Basic Life Support (BLS) / CPR certification (or willing to obtain)
• Comfortable administering meds, IV/PO documentation, and basic nursing procedures
• Excellent communication skills and a team-first attitude
• Previous experience in rehab, skilled nursing, or long-term care preferred

Education

• Graduate of an accredited Practical Nursing program (LPN)

Knowledge & Skills

• Medication administration and safe documentation practices
• Wound care basics and pressure injury prevention
• Ability to take and interpret vital signs and report changes
• Familiarity with electronic medical records and point-of-care charting
• Strong resident-centered care approach and empathy
• Time management and prioritization during busy shifts
