Roles & Responsibilities
- Provide compassionate direct patient care to residents in a skilled nursing setting
- Administer medications and treatments safely and accurately
- Monitor and document patient vital signs, status changes, and care plans
- Collaborate with RNs, therapists, and care staff to deliver coordinated care
- Perform wound care and assist with rehabilitative activities as directed
- Educate patients and families about care plans and follow-up needs
- Maintain a safe, clean environment and follow infection control procedures
- Participate in admissions, transfers, and discharge processes to ensure continuity of care
Qualifications
- Current LPN license (Massachusetts preferred)
- Graduation from an accredited practical nursing (LPN) program
- Current BLS/CPR certification or willingness to obtain
- Experience in long-term care or rehabilitation settings preferred
- Strong communication, assessment, and documentation skills
- Ability to work effectively as part of an interdisciplinary care team
Education
- Completion of an accredited practical nursing (LPN) program
- High school diploma or GED
Knowledge & Skills
- Medication administration and safe handling practices
- Basic nursing assessments and documentation
- Wound care and pressure injury prevention
- Familiarity with electronic medical records/charting
- Patient safety, falls prevention, and infection control
- Effective teamwork and clear communication with staff and families
