Roles & Responsibilities

• Evaluate patient mobility, strength, balance, and functional needs and develop individualized treatment plans
• Provide hands-on therapy, therapeutic exercise, gait training, and functional task practice to help patients reach goals
• Document patient progress accurately and timely in the medical record
• Collaborate with nurses, occupational therapists, speech therapists, case managers, and physicians to coordinate care
• Educate patients and families on home programs, safety strategies, and discharge planning
• Ensure compliance with facility policies, safety standards, and infection control protocols
• Support continuity of care across the Parkersburg and Willows sites for split assignments

Qualifications

• Current Physical Therapist license in West Virginia or eligible for licensure
• Degree in Physical Therapy (see education section) and relevant clinical training
• Basic Life Support (BLS) or CPR certification (or willingness to obtain prior to start)
• Experience in inpatient rehabilitation, outpatient, or skilled nursing settings preferred
• Strong communication, documentation, and teamwork skills
• Ability to travel between split sites (Parkersburg and Willows) as required by the assignment

Education

• Doctor of Physical Therapy (DPT) preferred
• Master's or Bachelor's degree in Physical Therapy or equivalent may be considered

Knowledge & Skills

• Skilled in therapeutic exercise, manual therapy techniques, and functional mobility training
• Knowledge of rehabilitation principles for adults with orthopedic, neurological, and medical diagnoses
• Proficient with assessment tools, outcome measures, and care planning
• Comfortable using electronic medical records and timely documentation workflows
• Strong time management and prioritization in fast-paced environments
• Ability to educate and engage patients and caregivers to achieve meaningful outcomes
