Roles & Responsibilities

• Assess patient mobility, balance, strength and functional needs to establish individualized plans of care
• Develop and implement evidence-based physical therapy treatment plans to improve patient outcomes
• Deliver hands-on therapy and therapeutic exercise, progressing interventions based on patient response
• Collaborate with interdisciplinary team members to coordinate comprehensive patient-centered care
• Document patient evaluations, progress notes and discharge summaries in a timely and accurate manner
• Educate patients and families on injury prevention, home exercise programs and safe mobility strategies
• Monitor outcomes and adjust treatment plans to meet functional goals and facility expectations
• Participate in team meetings and contribute to continuous quality improvement initiatives

Qualifications

• Valid Physical Therapist license (able to work in Maine or obtain temporary licensure prior to start)
• Graduation from an accredited physical therapy program
• Current BLS/CPR certification or willingness to obtain upon hire
• Comfort working in a fast-paced contract environment and adapting to varied caseloads
• Strong communication skills and a patient-centered approach
• Previous outpatient/rehab experience preferred but new grads with strong clinical skills are welcome

Education

• Doctor of Physical Therapy (DPT) preferred; Master of Physical Therapy considered

Knowledge & Skills

• Skilled in therapeutic exercise, manual therapy and mobility training
• Knowledge of functional outcome measures and documentation best practices
• Ability to develop individualized treatment plans and set measurable goals
• Familiarity with safety protocols for transfers and fall prevention
• Strong clinical reasoning, time management and interpersonal skills
