Roles & Responsibilities

• Provide skilled therapeutic interventions under the plan of care to help patients regain mobility and independence
• Assist with therapeutic exercises, gait training, transfers and use of assistive devices
• Monitor and document patient progress clearly and promptly in the medical record
• Collaborate with PTs, nurses and the rehab team to deliver coordinated, patient-centered care
• Educate patients and families on home programs, safety and strategies to maintain functional gains
• Maintain equipment, ensure a safe treatment environment, and adhere to infection control practices

Qualifications

• Licensed Physical Therapist Assistant (PTA) in New Jersey or able to obtain state licensure promptly
• Graduate of an accredited PTA program
• Current BLS/CPR certification
• Comfortable working with adult and geriatric populations in a rehab setting
• Strong communication and documentation skills; team player attitude
• Prior experience in outpatient, skilled nursing, or contract settings is a plus

Education

• Associate degree from an accredited Physical Therapist Assistant program (required)

Knowledge & Skills

• Skilled in therapeutic modalities, exercise progression and functional mobility training
• Proficient with patient assessment, treatment planning and measurement of outcomes
• Able to instruct and motivate patients and caregivers with clear, empathetic education
• Familiarity with electronic medical records and timely documentation
• Good time management, prioritization, and teamwork skills
