Roles & Responsibilities

• Evaluate patient mobility and functional status to create targeted care plans
• Develop and implement individualized therapy programs that restore independence
• Deliver skilled hands-on and therapeutic exercise interventions
• Train patients and caregivers in safe transfers, gait, and home programs
• Document progress accurately and communicate outcomes with the care team
• Collaborate with nurses, physicians, and rehab staff to optimize recovery
• Participate in discharge planning and transitions of care
• Contribute to quality improvement initiatives to elevate patient outcomes

Qualifications

• Licensed Physical Therapist or Physical Therapist Assistant in New Hampshire, or eligible to obtain state licensure
• Graduated from an accredited PT or PTA program
• BLS certification preferred or ability to obtain upon hire
• Strong clinical judgment, documentation skills, and patient-centered mindset
• Excellent communication and teamwork skills; compassionate approach to care

Education

• Physical Therapist: Doctor of Physical Therapy (DPT) or equivalent from an accredited program
• Physical Therapist Assistant: Associate degree or completion of an accredited PTA program

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise, and functional mobility training
• Experience with gait training, transfers, and assistive device recommendations
• Knowledge of rehabilitation modalities and safety/infection control practices
• Proficient documentation and basic electronic medical record comfort
• Ability to educate patients and families in clear, encouraging language
