Roles & Responsibilities

• Provide thorough evaluations and develop individualized treatment plans that help patients regain independence
• Deliver evidence-based therapeutic interventions focused on function, safety, and participation in daily activities
• Collaborate with nurses, PTs, SLPs, and the care team to coordinate holistic patient-centered care
• Train patients and families on adaptive techniques, home modifications, and safe functional mobility
• Document assessment findings, treatment plans, and progress in the facility EMR clearly and timely
• Recommend and fit assistive devices and adaptive equipment to maximize patient outcomes
• Participate in team meetings and contribute to quality improvement and discharge planning

Qualifications

• Licensed Occupational Therapist in Vermont or eligible for Vermont licensure (active license required for OT role)
• Certified Occupational Therapy Assistant (COTA) credential required if applying for the COTA role
• Basic Life Support (BLS/CPR) certification
• Comfortable documenting in an EMR and working within a multidisciplinary team
• Open to new graduates and experienced clinicians — this role will accept new grads
• Ability to start on the required start date (assignment begins 2025-12-29)

Education

• Occupational Therapist: Entry-level Master’s degree (MS/OTD) in Occupational Therapy or equivalent
• Certified Occupational Therapy Assistant: Completion of an accredited OTA/associate program

Knowledge & Skills

• Skilled in assessment and treatment planning for adult rehabilitation and function
• Knowledge of adaptive equipment, splinting basics, and activity analysis
• Strong clinical reasoning and goal-focused treatment approach
• Excellent communication skills with patients, families, and the care team
• Proficient documentation and time management to support heavy caseloads
• Ability to coach caregivers and provide patient-centered education
