Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (Day shift) with occasional weekends

Candidate Pay Rate: $82 per hour

Roles & Responsibilities
- Coordinate patient care across teams to ensure smooth transitions and timely discharges
- Perform utilization review and case management activities to support appropriate resource use
- Document assessments, care plans and utilization findings accurately in Cerner
- Collaborate with physicians, social workers and community partners to arrange post-discharge services
- Advocate for patients and families to help them navigate care options and benefits
- Monitor patient progress and update the care team to support safe, efficient care delivery
- Participate in team huddles and quality initiatives to improve care coordination processes
Qualifications
- Active California Registered Nurse (RN) license (required)
- BLS (Basic Life Support) certification (required)
- Minimum 2 years of RN experience (explicitly required)
- Experience in care coordination, case management or utilization review
- Familiarity with Cerner charting system
- Ability to work day shifts with occasional weekends
- Acute care experience preferred
Knowledge & Skills
- Proficient documentation and charting in Cerner
- Strong understanding of utilization review and case management principles
- Excellent communication and interdisciplinary collaboration skills
- Ability to prioritize tasks and manage a caseload effectively
- Comfort working in an acute care environment and handling fast-paced workflows
- Patient advocacy and discharge planning knowledge
