Roles & Responsibilities

• Coordinate patient care across teams to ensure smooth transitions and timely discharges
• Perform utilization review and case management activities to support appropriate resource use
• Document assessments, care plans and utilization findings accurately in Cerner
• Collaborate with physicians, social workers and community partners to arrange post-discharge services
• Advocate for patients and families to help them navigate care options and benefits
• Monitor patient progress and update the care team to support safe, efficient care delivery
• Participate in team huddles and quality initiatives to improve care coordination processes

Qualifications

• Active California Registered Nurse (RN) license (required)
• BLS (Basic Life Support) certification (required)
• Minimum 2 years of RN experience (explicitly required)
• Experience in care coordination, case management or utilization review
• Familiarity with Cerner charting system
• Ability to work day shifts with occasional weekends
• Acute care experience preferred

Education

• BSN not required

Knowledge & Skills

• Proficient documentation and charting in Cerner
• Strong understanding of utilization review and case management principles
• Excellent communication and interdisciplinary collaboration skills
• Ability to prioritize tasks and manage a caseload effectively
• Comfort working in an acute care environment and handling fast-paced workflows
• Patient advocacy and discharge planning knowledge
