Roles & Responsibilities

• Provide compassionate, patient-centered nursing care in a skilled rehabilitation setting
• Perform thorough assessments and communicate changes in patient condition to the care team
• Administer medications and treatments safely and accurately following facility protocols
• Document care, interventions and patient progress clearly and timely in the medical record
• Collaborate with therapists, physicians and family members to support each patient’s recovery goals
• Assist with admissions, discharges and care transitions to ensure continuity of care
• Adhere to infection control standards and contribute to a safe clinical environment

Qualifications

• Current LPN or RN license in good standing (Maryland licensure or compact/eligible)
• Basic Life Support (BLS) certification or willingness to obtain prior to start
• Current immunizations and TB screening per facility policy
• Previous experience in long-term care, skilled nursing, or rehabilitation preferred
• Strong communication skills and ability to work collaboratively in a multidisciplinary team

Education

• Graduate of an accredited Practical Nursing or Registered Nursing program
• LPN: Practical Nursing diploma or certificate; RN: ADN or BSN

Knowledge & Skills

• Clinical assessment and medication administration expertise
• Skilled with electronic charting and documentation best practices
• Familiarity with wound care, IV basics (as applicable), and rehab nursing principles
• Ability to prioritize tasks in a fast-paced environment while maintaining compassionate care
• Effective teamwork, time management and patient advocacy skills
