Roles & Responsibilities

• Provide compassionate hospice and home care nursing to patients in long-term care and home settings
• Perform thorough patient assessments and create individualized care plans
• Manage symptoms and provide palliative interventions to improve comfort and quality of life
• Administer medications, treatments, and necessary nursing interventions safely and accurately
• Coordinate care with interdisciplinary teams, families, and community resources
• Educate patients and caregivers on disease progression, symptom management, and care techniques
• Maintain timely, accurate clinical documentation and complete required competency testing
• Adhere to infection control protocols and organizational policies to keep patients safe
• Support end-of-life planning and provide compassionate family support during transitions

Qualifications

• Active Registered Nurse (RN) license (license/certificate verification required)
• BLS certification (required)
• Experience or comfort working in hospice, home care, or long-term care settings preferred
• Completion of Trinity specialty competency testing and required pre-placement screenings
• Up-to-date immunizations and health screenings (PPD/IGRA, MMR, Hep B, Varicella, Influenza, Tdap, COVID-19)
• Successful completion of Trinity 10-panel drug screen, respirator evaluation, fit test, and color vision screening
• Strong clinical assessment, communication, and critical thinking skills

Knowledge & Skills

• Knowledge of hospice and palliative care principles and best practices
• Home safety assessment and the ability to provide care in non-hospital settings
• Competence with medication administration and symptom management
• Comfortable working independently and coordinating with interdisciplinary teams
• Proficient documentation skills and adherence to regulatory requirements
• Excellent communication and family education skills
• Ability to manage time effectively and prioritize patient needs
