Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to older adults in a long-term care/PACE setting
• Perform nursing assessments and develop or update individualized care plans
• Administer medications and treatments safely and document per facility policy
• Monitor patient status, recognize changes, and escalate concerns to the care team
• Collaborate with interdisciplinary team members to support holistic care
• Educate patients and families on care plans, medications, and self-care strategies
• Maintain accurate, timely documentation and complete required checklists and competencies
• Adhere to infection control practices and facility policies to keep patients safe

Qualifications

• Active Registered Nurse license (RN) as required by the employer
• Current BLS certification
• Experience or comfort working in long-term care or geriatric settings (PACE experience a plus)
• Completion of Trinity Skills Checklist and facility competency requirements
• Up-to-date immunizations and health screenings (2-Step PPD or IGRA, MMR, Hepatitis B, Varicella, Tdap, COVID-19, Influenza)
• Successful Trinity 10 Panel Drug Screen and license/certificate verification
• Excellent communication skills and ability to work collaboratively with a multidisciplinary team
• Willingness to work per diem shifts and adapt to variable scheduling

Knowledge & Skills

• Strong clinical assessment skills for older adult populations
• Medication administration and documentation best practices
• Familiarity with long-term care workflows and care planning
• Infection prevention and control procedures
• Comfort working within an interdisciplinary team model (PACE environment)
• Basic computer and electronic health record documentation skills
• Ability to prioritize, triage, and escalate clinical concerns appropriately
• Dependability, professionalism, and patient-centered bedside manner
