Shift Type: Per diem pool — variable shifts based on facility needs (day and night coverage possible); schedule varies and is assigned per facility demand

Roles & Responsibilities
- Assess and monitor residents' clinical status, identifying changes and intervening promptly
- Administer medications and treatments accurately and safely according to protocols
- Develop and update individualized care plans in collaboration with interdisciplinary team members
- Provide compassionate, resident-centered care that preserves dignity and independence
- Document assessments, interventions, and outcomes clearly in the medical record
- Educate residents and families about care plans, medications, and follow-up needs
- Participate in infection prevention, quality improvement, and safety initiatives
Qualifications
- Active Registered Nurse (RN) license in good standing
- Current BLS certification
- Long-term care (LTC) experience preferred
- Ability to complete Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
- Willingness to complete required health screenings: 2-step PPD or IGRA, MMR, Hepatitis B, Varicella, Tdap, COVID-19 and Influenza vaccinations as required
- Successful completion of fit test, Trinity 10-panel drug screen, respirator evaluation, and license/certificate verification
- Strong communication skills and ability to work on a collaborative team
Education
- Active RN license (required)
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred
Knowledge & Skills
- Knowledge of long-term care best practices and geriatric nursing
- Medication administration and charting proficiency
- Skilled in wound care, pressure injury prevention, and chronic condition management
- Familiarity with infection prevention and control procedures
- Comfortable using electronic medical records for documentation
- Excellent clinical assessment and critical thinking skills
- Compassionate patient and family education abilities
