Roles & Responsibilities

• Assess and monitor residents' clinical status, identifying changes and intervening promptly
• Administer medications and treatments accurately and safely according to protocols
• Develop and update individualized care plans in collaboration with interdisciplinary team members
• Provide compassionate, resident-centered care that preserves dignity and independence
• Document assessments, interventions, and outcomes clearly in the medical record
• Educate residents and families about care plans, medications, and follow-up needs
• Participate in infection prevention, quality improvement, and safety initiatives

Qualifications

• Active Registered Nurse (RN) license in good standing
• Current BLS certification
• Long-term care (LTC) experience preferred
• Ability to complete Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
• Willingness to complete required health screenings: 2-step PPD or IGRA, MMR, Hepatitis B, Varicella, Tdap, COVID-19 and Influenza vaccinations as required
• Successful completion of fit test, Trinity 10-panel drug screen, respirator evaluation, and license/certificate verification
• Strong communication skills and ability to work on a collaborative team

Education

• Active RN license (required)
• Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred

Knowledge & Skills

• Knowledge of long-term care best practices and geriatric nursing
• Medication administration and charting proficiency
• Skilled in wound care, pressure injury prevention, and chronic condition management
• Familiarity with infection prevention and control procedures
• Comfortable using electronic medical records for documentation
• Excellent clinical assessment and critical thinking skills
• Compassionate patient and family education abilities
