Roles & Responsibilities

• Provide compassionate, hands-on care to residents in a Skilled Nursing Facility
• Assist with activities of daily living including bathing, dressing, toileting, and mobility
• Monitor resident condition and report changes promptly to the care team
• Support safe transfers and mobility, using proper body mechanics and assistive devices
• Maintain accurate documentation of care and observations
• Follow infection control, safety protocols, and facility policies at all times
• Collaborate with nurses and interdisciplinary staff to deliver personalized resident care

Qualifications

• Current/state-certified CNA credential in good standing
• Required immunizations and health documentation: MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine, Influenza vaccine, and a current Physical/Health Statement
• Negative Trinity 10 Panel Drug Screen (per facility requirement)
• Comfort working in a Skilled Nursing Facility (SNF) environment and adapting to varied assignments
• Strong communication skills and a resident-centered attitude
• Willingness to complete facility-specific onboarding and trainings

Education

• Completion of a state-approved CNA program or equivalent training
• High school diploma or GED preferred

Knowledge & Skills

• Knowledge of basic nursing care and activities of daily living
• Familiarity with infection control procedures and PPE use
• Ability to follow care plans and document observations clearly
• Safe resident handling and transfer techniques
• Teamwork mindset and ability to communicate with clinical staff and families
• Time management and adaptability in fast-moving care settings
