Roles & Responsibilities

• Provide compassionate, patient-centered nursing care for long-term care and PACE participants
• Administer medications and treatments accurately and safely
• Monitor resident health status and promptly report changes to the care team
• Document assessments, interventions, and care plans in the medical record with clarity
• Coordinate care with interdisciplinary team members to ensure holistic support
• Educate participants and families about care plans, medications, and health promotion
• Support infection control practices, immunization requirements, and workplace safety
• Participate in Trinity skills checklist and required competency validations

Qualifications

• Active Registered Nurse (RN) licensure (state-required licensure/currently valid)
• BLS certification (required)
• Experience in long-term care or geriatric nursing preferred
• Successful completion of Trinity Skills Checklist and License/Certificate Verification
• Up-to-date immunizations and health screenings (Fit Test, 2-step PPD or IGRA, MMR, Hep B, Varicella, Tdap, COVID-19, Influenza)
• Clear Trinity 10-panel drug screen and recent physical/health statement

Education

• Graduate of an accredited nursing program (ADN or BSN)
• Current RN licensure required

Knowledge & Skills

• Strong clinical assessment and medication administration skills
• Knowledge of long-term care practices and geriatric care needs
• Comfortable with electronic medical records and documentation best practices
• Excellent communication and teamwork skills with interdisciplinary staff
• Ability to prioritize, triage, and respond to changing resident needs
• Familiarity with infection control and vaccine requirements
