Roles & Responsibilities
- Provide compassionate, patient-centered nursing care for long-term care and PACE participants
- Administer medications and treatments accurately and safely
- Monitor resident health status and promptly report changes to the care team
- Document assessments, interventions, and care plans in the medical record with clarity
- Coordinate care with interdisciplinary team members to ensure holistic support
- Educate participants and families about care plans, medications, and health promotion
- Support infection control practices, immunization requirements, and workplace safety
- Participate in Trinity skills checklist and required competency validations
Qualifications
- Active Registered Nurse (RN) licensure (state-required licensure/currently valid)
- BLS certification (required)
- Experience in long-term care or geriatric nursing preferred
- Successful completion of Trinity Skills Checklist and License/Certificate Verification
- Up-to-date immunizations and health screenings (Fit Test, 2-step PPD or IGRA, MMR, Hep B, Varicella, Tdap, COVID-19, Influenza)
- Clear Trinity 10-panel drug screen and recent physical/health statement
Education
- Graduate of an accredited nursing program (ADN or BSN)
- Current RN licensure required
Knowledge & Skills
- Strong clinical assessment and medication administration skills
- Knowledge of long-term care practices and geriatric care needs
- Comfortable with electronic medical records and documentation best practices
- Excellent communication and teamwork skills with interdisciplinary staff
- Ability to prioritize, triage, and respond to changing resident needs
- Familiarity with infection control and vaccine requirements
