Shift Type: Per diem (PD Pool) — variable shifts as scheduled; assignments may include day, evening or night shifts and will include weekday and weekend opportunities (start/end times vary by assignment)

Roles & Responsibilities
- Provide compassionate, patient-centered personal care to hospice and home care patients
- Assist with activities of daily living (bathing, grooming, mobility, feeding) while preserving dignity
- Monitor and report changes in patient condition to the interdisciplinary team promptly
- Support comfort measures and follow individualized care plans for end-of-life needs
- Complete accurate documentation and required checklists (including Trinity Skills Checklist)
- Adhere to infection control practices, immunization and fit-test requirements
- Collaborate respectfully with nurses, social workers, and hospice clinicians to ensure continuity of care
- Participate in mandated health screenings and drug testing as part of onboarding and ongoing compliance
Qualifications
- Active CNA certification in the state of Maryland (required)
- BLS certification (required)
- NRP and TNCC certifications listed as required by the order
- Successful License/Certificate Verification through employer systems
- Completion of Trinity Skills Checklist prior to assignment
- Current fit test and required immunizations (PPD/IGRA, MMR, Hep B, Varicella, Tdap, COVID-19, Influenza)
- Clear Trinity 10-panel drug screen and current physical/health statement
- Comfort working in home settings and providing end-of-life supportive care
Education
- High school diploma or GED preferred
- Current Certified Nursing Assistant (CNA) credential
Knowledge & Skills
- Compassionate bedside manner and strong communication with patients and families
- Basic clinical skills for assisting with ADLs and patient mobility
- Understanding of hospice philosophy and palliative care basics
- Ability to follow care plans and report changes in condition accurately
- Familiarity with infection control practices and immunization protocols
- Comfort with documentation and checklist-driven competency verification
