Roles & Responsibilities

• Provide compassionate, patient-centered long-term care to residents in the LTC setting
• Perform thorough assessments and create clear, prioritized care plans
• Administer medications and treatments safely and accurately
• Document clinical care and observations in the patient record in a timely manner
• Collaborate with nursing staff, providers, and support teams to optimize patient outcomes
• Follow infection control protocols and participate in safety initiatives
• Complete required Trinity specialty competency testing and skills checklist to demonstrate clinical readiness

Qualifications

• Active RN license (ability to practice in Massachusetts required or compact state where applicable)
• Current BLS certification
• Experience in long-term care nursing or demonstrated comfort caring for LTC residents
• Ability to complete Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
• Must be able to pass Trinity 10-panel drug screen, fit test, respirator evaluation, and required immunizations
• Current license/certification verification and health documentation as listed (MMR, Hep B, Varicella, Tdap, COVID-19, Influenza)
• Reliable communication and teamwork skills with a professional bedside manner

Knowledge & Skills

• Strong patient care fundamentals and LTC best practices
• Medication administration and documentation accuracy
• Basic Life Support and emergency response procedures
• Infection control and resident safety protocols
• Charting and electronic health record familiarity
• Effective communication with residents, families, and interdisciplinary teams
• Time management and prioritization in a fast-paced LTC environment
• Comfort with competency-based testing and skills validation processes
