Roles & Responsibilities
- Provide compassionate, person-centered care to home care and hospice patients
- Assist patients with activities of daily living including bathing, dressing, and mobility
- Support comfort measures and dignity-focused care for hospice clients
- Accurately document patient status, interventions, and observations in the record
- Communicate clearly with family members and the interdisciplinary care team
- Follow infection prevention protocols and complete required screenings and fit testing
- Participate in Trinity specialty competency testing and ongoing skill development
Qualifications
- Current CNA certification or state-approved nursing assistant credential
- BLS certification (required)
- Successful completion of Trinity specialty-specific competency testing
- License/certificate verification as part of onboarding
- Ability to pass required health screenings and clearances: fit test, 2-step PPD or IGRA, influenza vaccine, Trinity 10 panel drug screen, MMR, Hepatitis B, Varicella, physical/health statement, respirator evaluation, color vision check, Tdap, and COVID-19 vaccine
Education
- High school diploma or equivalent preferred
- Completion of an approved CNA program and current CNA credential
Knowledge & Skills
- Knowledge of home care and hospice care principles and compassionate end-of-life support
- Strong patient safety and infection control practices
- Skilled in basic nursing care and ADL assistance
- Comfortable documenting care accurately and communicating with families
- Ability to follow care plans and collaborate with interdisciplinary teams
- Professionalism, empathy, and strong observational skills
