Roles & Responsibilities

• Provide compassionate, person-centered care to home care and hospice patients
• Assist patients with activities of daily living including bathing, dressing, and mobility
• Support comfort measures and dignity-focused care for hospice clients
• Accurately document patient status, interventions, and observations in the record
• Communicate clearly with family members and the interdisciplinary care team
• Follow infection prevention protocols and complete required screenings and fit testing
• Participate in Trinity specialty competency testing and ongoing skill development

Qualifications

• Current CNA certification or state-approved nursing assistant credential
• BLS certification (required)
• Successful completion of Trinity specialty-specific competency testing
• License/certificate verification as part of onboarding
• Ability to pass required health screenings and clearances: fit test, 2-step PPD or IGRA, influenza vaccine, Trinity 10 panel drug screen, MMR, Hepatitis B, Varicella, physical/health statement, respirator evaluation, color vision check, Tdap, and COVID-19 vaccine

Education

• High school diploma or equivalent preferred
• Completion of an approved CNA program and current CNA credential

Knowledge & Skills

• Knowledge of home care and hospice care principles and compassionate end-of-life support
• Strong patient safety and infection control practices
• Skilled in basic nursing care and ADL assistance
• Comfortable documenting care accurately and communicating with families
• Ability to follow care plans and collaborate with interdisciplinary teams
• Professionalism, empathy, and strong observational skills
