Roles & Responsibilities
- Provide compassionate assistance with activities of daily living including bathing, dressing, toileting, and feeding
- Monitor and record vital signs and report changes in resident condition to the clinical team
- Assist with safe transfers and mobility, using proper body mechanics and lifting equipment
- Follow individualized care plans and document care accurately and timely
- Support infection prevention practices and adhere to facility health requirements
- Collaborate with nursing staff and interdisciplinary team to promote resident comfort and dignity
- Maintain a clean, safe environment and participate in shift handoffs to ensure continuity of care
Qualifications
- Active Certified Nursing Assistant (CNA) credential (state-issued)
- Current BLS (Basic Life Support) certification
- Ability to pass Trinity Specialty Specific Competency Testing and License/Certificate Verification
- Up-to-date health requirements: Fit Test, 2-step PPD or IGRA (Quantiferon/T-Spot), influenza vaccine, MMR, Hepatitis B, Varicella, Tdap, and COVID-19 vaccine as required
- Negative Trinity 10 Panel Drug Screen and completed Physical/Health Statement
- Completion of Trinity Respirator Evaluation and Trinity Color Vision testing where applicable
- Strong communication skills and a patient-centered attitude
Education
- Certified Nursing Assistant (CNA) certification

- Basic vital sign measurement and documentation
- Infection prevention and standard precautions
- Safe transfer and mobility assistance
- Accurate charting and following care plans
