Roles & Responsibilities

• Provide compassionate personal care to residents, assisting with activities of daily living
• Assist with bathing, grooming, dressing, toileting, and mobility support
• Take and record vital signs and communicate changes in condition to the care team
• Perform hands-on care while following infection control and safety procedures
• Complete accurate and timely documentation of resident care and observations
• Support mealtime assistance and nutritional/feeding needs as required
• Collaborate with nurses, therapists, and support staff to promote resident wellbeing
• Participate in competency testing and training to maintain high standards of care

Qualifications

• Certified Nursing Assistant (CNA) certification required
• Basic Life Support (BLS) certification required
• License/Certificate verification and completion of Trinity-specific competency testing
• Must be able to complete required health screenings: Fit test, TB screening (2-step PPD or IGRA), MMR, Hep B, Varicella, Tdap, and COVID-19 vaccine as applicable
• Clean Trinity 10 panel drug screen and physical/health statement required
• Ability to pass Trinity respirator evaluation and color vision screening as applicable
• Strong communication skills and a caring, patient-centered approach

Education

• Certified Nursing Assistant (CNA) certification (required)
• High school diploma or equivalent preferred

Knowledge & Skills

• Knowledge of resident care best practices and infection control
• Comfortable assisting with mobility, transfers, and safe use of equipment
• Basic clinical skills including taking vitals and observing resident condition
• Reliable documentation and charting habits
• Ability to follow care plans and communicate changes to the nursing team
• Compassionate interpersonal skills and patience with older adults
