Roles & Responsibilities

• Provide compassionate hands-on care to PACE participants, assisting with activities of daily living
• Monitor and report changes in participant condition to nursing staff and providers
• Assist with safe transfers, mobility, and personal hygiene while promoting independence
• Follow individualized care plans and document care accurately using required checklists
• Support a team-based approach to care, communicating clearly with interdisciplinary staff
• Adhere to infection control practices and maintain a safe environment for participants
• Participate in Trinity Skills Checklist and ongoing competency validation

Qualifications

• Current CNA certification in the state of Florida
• Basic Life Support (BLS) certification required
• Completion of Trinity Skills Checklist (required prior to or during onboarding)
• License/certification verification and clear background check
• Ability to complete health requirements: 2-step PPD or IGRA, MMR, Hep B, Varicella, Tdap, COVID-19 vaccine, and annual influenza vaccine
• Successful Trinity 10-panel drug screen and physical/health statement
• Strong communication skills and a patient-first mindset

Knowledge & Skills

• Skilled in assisting with ADLs and basic patient care
• Knowledge of infection control and safe patient handling techniques
• Comfortable with documentation and following care plans
• Ability to observe, assess, and escalate changes in participant condition
• Teamwork-oriented with good interpersonal and communication skills
• Adaptable to variable schedules and changing care environments
