Roles & Responsibilities
- Provide compassionate hands-on care to PACE participants, assisting with activities of daily living
- Monitor and report changes in participant condition to nursing staff and providers
- Assist with safe transfers, mobility, and personal hygiene while promoting independence
- Follow individualized care plans and document care accurately using required checklists
- Support a team-based approach to care, communicating clearly with interdisciplinary staff
- Adhere to infection control practices and maintain a safe environment for participants
- Participate in Trinity Skills Checklist and ongoing competency validation
Qualifications
- Current CNA certification in the state of Florida
- Basic Life Support (BLS) certification required
- Completion of Trinity Skills Checklist (required prior to or during onboarding)
- License/certification verification and clear background check
- Ability to complete health requirements: 2-step PPD or IGRA, MMR, Hep B, Varicella, Tdap, COVID-19 vaccine, and annual influenza vaccine
- Successful Trinity 10-panel drug screen and physical/health statement
- Strong communication skills and a patient-first mindset
Knowledge & Skills
- Skilled in assisting with ADLs and basic patient care
- Knowledge of infection control and safe patient handling techniques
- Comfortable with documentation and following care plans
- Ability to observe, assess, and escalate changes in participant condition
- Teamwork-oriented with good interpersonal and communication skills
- Adaptable to variable schedules and changing care environments
