Roles & Responsibilities
- Provide compassionate personal care to residents in the Saint Francis LIFE program
- Assist with activities of daily living (bathing, dressing, toileting, mobility)
- Monitor and report changes in resident condition to the care team
- Perform assigned clinical tasks according to care plans and facility policies
- Document care accurately and timely using facility charting systems
- Support resident engagement in program activities and promote independence
- Collaborate with interdisciplinary team members to deliver person-centered care
Qualifications
- Active CNA certification (required) — License/Certificate Verification will be completed
- BLS (Basic Life Support) certification (required)
- Completion of required health screening and immunizations: Fit Test, Trinity Respirator Evaluation, Trinity 10-Panel Drug Screen, 2-Step PPD or IGRA (Quantiferon), MMR, Hepatitis B, Varicella, Tdap, Physical/Health Statement, COVID-19 vaccine, Influenza vaccine
- Ability to pass facility onboarding checks and background screening
- Comfort working in a per diem pool and adapting to varied shift assignments
Education
- Active CNA certification (required)
- High school diploma or equivalent (preferred)
Knowledge & Skills
- Fundamental resident care skills and safe transfer techniques
- Basic assessment skills: observing and reporting changes in condition
- Infection prevention and control practices
- Strong communication and teamwork with nursing and support staff
- Reliable documentation practices and attention to detail
- Professionalism, empathy, and a resident-centered approach
