Roles & Responsibilities
- Provide compassionate personal care and assistance with activities of daily living (ADLs) for residents
- Perform essential clinical tasks and follow the Trinity Skills Checklist to ensure consistent, high-quality care
- Maintain accurate and timely documentation of resident care and observations
- Follow infection prevention protocols and maintain immunization and screening compliance
- Communicate clearly and respectfully with residents, families, and the interdisciplinary care team
- Participate in license/certificate verification and required health screenings
- Support a positive resident experience by promoting dignity, independence, and safety
Qualifications
- Active Certified Nursing Assistant (CNA) credential
- Current Basic Life Support (BLS) certification
- Successful completion of License/Certificate Verification and Trinity Skills Checklist
- Ability to complete required health and screening items: Fit Test, Trinity Respirator Evaluation, Trinity 10 Panel Drug Screen
- Up-to-date immunizations or proof of immunity: MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine, Influenza
- 2-step PPD or IGRA (Quantiferon) and an acceptable Physical/Health Statement
Education
- High school diploma or equivalent preferred
Knowledge & Skills
- Solid understanding of activities of daily living (ADLs) and resident-centered care
- Familiarity with infection control practices and immunization requirements
- Comfortable with clinical screening procedures (fit testing, respirator evaluation, drug screening)
- Strong communication and interpersonal skills with residents and care teams
- Reliable documentation and basic clinical observation skills
- Ability to follow established checklists and facility protocols (Trinity Skills Checklist)
