Roles & Responsibilities

• Provide compassionate, patient-centered care in clients' homes, assisting with activities of daily living
• Follow care plans and communicate changes to the care team promptly and professionally
• Perform required clinical tasks according to Trinity competency standards and facility protocols
• Maintain clear, accurate documentation of care and patient status
• Demonstrate infection control practices, complete fit testing, and follow vaccination and screening requirements
• Support safe transitions of care and collaborate with nurses and clinicians to optimize patient outcomes
• Help create a comfortable, respectful environment for patients and their families

Qualifications

• Current Certified Nursing Assistant (CNA) credential in good standing
• BLS certification required
• Successful completion of Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
• License/certificate verification and ability to meet employer background checks
• Completion of required health screenings: 2-step PPD or IGRA (Quantiferon), COVID-19 questionnaire, physical/health statement
• Required immunizations and screenings: COVID-19 vaccine, MMR, Hepatitis B, Influenza vaccine, Varicella, Tdap
• Trinity 10 Panel Drug Screen and Trinity Color Vision check as part of onboarding
• Fit test clearance as applicable for assigned tasks

Education

• High school diploma or equivalent preferred
• Current CNA certification required

Knowledge & Skills

• Comfort working independently in home care settings
• Strong patient communication and family-centered care approach
• Knowledge of infection prevention and safe patient handling
• Able to follow detailed care plans and document accurately
• Familiarity with Trinity competency standards and checklists
• Attention to detail for compliance items like drug screens, immunizations, and fit testing
• Reliable time management and professional bedside manner
