Roles & Responsibilities

• Provide compassionate personal care and assistance with activities of daily living for home-based patients
• Perform patient mobility support, transfers, and safe use of assistive devices
• Follow individualized care plans and document patient status clearly and promptly
• Monitor basic vital signs and report changes to the care team
• Support infection prevention practices, including PPE and hygiene protocols
• Build trusting relationships with patients and family members through respectful communication
• Complete required Trinity Skills Checklist and participate in required training and evaluations

Qualifications

• Certified Nursing Assistant (CNA) — active certification required
• Basic Life Support (BLS) certification required
• Completion of Trinity Skills Checklist and successful license/certificate verification
• Ability to pass required health screenings: 2-step PPD or IGRA (Quantiferon), fit test, Trinity 10-panel drug screen, respirator evaluation
• Up-to-date immunizations: MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine, and annual influenza vaccine as required
• Reliable transportation and ability to travel to patient homes as assigned
• Strong communication skills and a patient-centered attitude

Education

• High school diploma or GED required
• Current CNA certification required

Knowledge & Skills

• Hands-on patient care and personal care assistance
• Safe patient handling and transfer techniques
• Basic vital signs monitoring and documentation
• Infection control and proper PPE use
• Clear, timely clinical documentation and reporting
• Time management and ability to prioritize patient needs
• Comfort working independently in home settings and collaborating with a clinical team
