Roles & Responsibilities
- Provide compassionate personal care and assistance with activities of daily living for home-based patients
- Perform patient mobility support, transfers, and safe use of assistive devices
- Follow individualized care plans and document patient status clearly and promptly
- Monitor basic vital signs and report changes to the care team
- Support infection prevention practices, including PPE and hygiene protocols
- Build trusting relationships with patients and family members through respectful communication
- Complete required Trinity Skills Checklist and participate in required training and evaluations
Qualifications
- Certified Nursing Assistant (CNA) — active certification required
- Basic Life Support (BLS) certification required
- Completion of Trinity Skills Checklist and successful license/certificate verification
- Ability to pass required health screenings: 2-step PPD or IGRA (Quantiferon), fit test, Trinity 10-panel drug screen, respirator evaluation
- Up-to-date immunizations: MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine, and annual influenza vaccine as required
- Reliable transportation and ability to travel to patient homes as assigned
- Strong communication skills and a patient-centered attitude
Education
- High school diploma or GED required
- Current CNA certification required
Knowledge & Skills
- Hands-on patient care and personal care assistance
- Safe patient handling and transfer techniques
- Basic vital signs monitoring and documentation
- Infection control and proper PPE use
- Clear, timely clinical documentation and reporting
- Time management and ability to prioritize patient needs
- Comfort working independently in home settings and collaborating with a clinical team
