Roles & Responsibilities
- Assist residents with activities of daily living (bathing, dressing, toileting) with compassion and respect
- Support rehabilitation goals by assisting therapists and reinforcing care plans during daily routines
- Monitor and record vital signs, fluid intake/output, and observable changes in condition
- Maintain accurate, timely documentation and communicate observations to nursing staff
- Ensure a clean, safe environment by following infection control and safety protocols
- Assist with safe patient transfers and mobility using proper body mechanics and equipment
- Build rapport with residents and families to provide person-centered, dignified care
Qualifications
- Valid CNA certification (required) and verifiable license/certificate
- Basic Life Support (BLS) certification (required)
- Ability to pass Trinity 10-panel drug screen and required health screenings
- Compliance with required immunizations and screening: COVID-19 vaccine, Influenza vaccine, MMR, Hepatitis B, Varicella, Tdap, TB testing or questionnaire, and fit test
- Current physical/health statement and Trinity color vision check as required
- Reliable, professional, with good communication and teamwork skills
- Previous experience in long-term care or rehab settings is a plus
Education
- CNA certification (required)
- High school diploma or equivalent (preferred)
Knowledge & Skills
- Skilled in assisting with ADLs and basic patient care
- Knowledge of infection prevention and proper PPE use
- Ability to take and document vital signs accurately
- Familiarity with basic rehab activities and mobility assistance
- Strong communication skills with residents, families, and clinical team
- Comfortable working in a fast-paced residential or rehab environment
- Attention to detail for documentation and following care plans
- Team-oriented mindset and commitment to resident dignity and safety
