Roles & Responsibilities
- Provide compassionate, hands-on nursing care to patients in their homes
- Perform focused assessments and monitor patient status, reporting changes
- Administer medications and treatments safely and accurately
- Document patient care and observations thoroughly in the electronic record
- Collaborate with interdisciplinary team members to coordinate care plans
- Educate patients and families on care plans, self-care, and safety at home
- Follow infection control and safety protocols to protect patients and staff
- Maintain competency through Trinity-specific testing and skills checklists
Qualifications
- Current Licensed Practical Nurse (LPN) license (valid for practice)
- Basic Life Support (BLS) certification required
- NIHSS certification required
- Successful completion of Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
- License and certificate verification as part of onboarding
- Required health screenings and immunizations: fit test, 10-panel drug screen, Tdap, physical/health statement, color vision check, 2-step PPD or IGRA, COVID questionnaire and vaccine, MMR, Hepatitis B, influenza, Varicella
- Comfort working independently in patient homes and excellent communication skills
- Home care experience preferred but not strictly required
Education
- Graduate of an accredited practical nursing program (LPN/Vocational Nursing diploma)
- High school diploma or GED required
Knowledge & Skills
- Skilled in patient assessment and care planning in the home environment
- Medication administration and basic wound care
- Strong documentation skills and familiarity with electronic health records
- Infection control best practices and home safety assessments
- Ability to prioritize tasks and manage time effectively during visits
- Excellent interpersonal skills for patient and family education
- Adaptability to varied patient needs and flexible scheduling
