Roles & Responsibilities
- Assess patient function and create individualized treatment plans to restore mobility and independence
- Deliver skilled therapeutic interventions using evidence-based techniques and therapeutic modalities
- Document progress and outcomes clearly and promptly in the patient record
- Collaborate with interdisciplinary team members to coordinate holistic patient care
- Educate patients and families on home exercise programs and safe mobility strategies
- Monitor patient response and adjust treatment plans to optimize recovery
- Support a culture of safety through fall prevention, proper body mechanics and environment checks
Qualifications
- Licensed Physical Therapist (state licensure required; ability to obtain NJ license if not currently licensed)
- Current CPR/BLS certification
- Experience in inpatient or rehab settings preferred
- Strong communication and documentation skills
- Ability to work independently within a contract assignment and collaborate with facility staff
Education
- Doctor of Physical Therapy (DPT) or equivalent degree in physical therapy preferred
Knowledge & Skills
- Skilled in evaluating mobility, strength and functional deficits
- Proficient with therapeutic exercise, gait training and balance interventions
- Familiarity with common rehab diagnoses and post-acute care workflows
- Comfortable using electronic health records for documentation
- Excellent teamwork, time management and patient education skills
