Roles & Responsibilities

• Provide compassionate direct patient care and assist residents with activities of daily living
• Assist with safe transfers, ambulation, bathing, dressing and toileting while promoting dignity
• Take and record vital signs and observe residents for changes in condition
• Document care accurately and communicate observations to the nursing team
• Support safe, timely admissions, discharges and resident transitions
• Follow infection control practices and maintain a clean, respectful environment
• Collaborate with nurses and interdisciplinary team members to meet resident goals
• Build trusting relationships with residents and families through respectful communication

Qualifications

• Active Certified Nursing Assistant (CNA) credential
• Current CPR/BLS certification or willingness to obtain upon hire
• Comfortable working 12-hour night shifts (5:00 PM – 5:00 AM) on the scheduled days
• Ability to provide patient-centered care in a long-term care or rehabilitation setting
• Strong communication skills and a team-first attitude
• Must meet facility background and health requirements (immunizations, screenings) as required

Education

• High school diploma or GED preferred

Knowledge & Skills

• Hands-on experience with ADLs, transfers and mobility assistance
• Accurate vital signs measurement and basic clinical observation
• Skilled in documentation and care reporting
• Infection control best practices and safe patient handling
• Time management for busy 12-hour shifts
• Empathy, patience and excellent communication with residents and families
