Roles & Responsibilities
- Evaluate patients' functional status and develop individualized treatment plans
- Deliver hands-on therapy and therapeutic exercise to drive measurable progress
- Monitor and document patient progress thoroughly and accurately in the medical record
- Collaborate with nurses, physicians and the rehab team to coordinate care and discharge planning
- Educate patients and families on home programs and strategies to maximize independence
- Adapt treatment plans based on ongoing assessment and patient goals to ensure outcomes
Qualifications
- Valid Physical Therapist license in New Hampshire or ability to obtain prior to start
- Current BLS/CPR certification
- Prior experience in rehabilitation, outpatient or inpatient settings preferred
- Strong communication skills and a patient-centered approach
- Ability to pass any facility onboarding requirements and background checks
Education
- Doctor of Physical Therapy (DPT) or Master’s degree in Physical Therapy
Knowledge & Skills
- Skilled in musculoskeletal and neuromuscular evaluation and manual therapy techniques
- Proficient with therapeutic exercise, gait training and mobility assistance
- Comfortable documenting in electronic medical records
- Strong clinical reasoning and goal-setting skills
- Team-oriented mindset with good time management and organizational skills
