Roles & Responsibilities

• Assess client needs and strengths to create client-centered care plans
• Coordinate services and collaborate with clinical teams, families, and community resources
• Provide counseling, crisis intervention, and supportive guidance to residents and families
• Document assessments, interventions, and outcomes accurately and timely
• Advocate for clients’ rights and access to appropriate services and supports
• Facilitate discharge planning and transitions of care with attention to safety and continuity
• Participate in interdisciplinary team meetings to improve quality of care and resident outcomes

Qualifications

• Licensed Social Worker (LMSW, LCSW, or state equivalent) or eligible for licensure in Maine
• Bachelor’s degree in Social Work (BSW) required; MSW preferred
• Experience in long-term care, behavioral health, or community case management is a plus
• Strong clinical assessment, documentation, and case management skills
• Comfortable working as part of an interdisciplinary team and advocating for clients
• Ability to pass required background checks and meet facility onboarding requirements

Education

• BSW (Bachelor of Social Work) required; MSW (Master of Social Work) preferred

Knowledge & Skills

• Knowledge of case management, psychosocial assessment, and care planning
• Skilled in crisis intervention and de-escalation techniques
• Excellent verbal and written communication for effective documentation and team coordination
• Familiarity with community resources, discharge planning, and referral processes
• Good organizational skills and the ability to manage a varied caseload
