Roles & Responsibilities
- Assess client needs and strengths to create client-centered care plans
- Coordinate services and collaborate with clinical teams, families, and community resources
- Provide counseling, crisis intervention, and supportive guidance to residents and families
- Document assessments, interventions, and outcomes accurately and timely
- Advocate for clients’ rights and access to appropriate services and supports
- Facilitate discharge planning and transitions of care with attention to safety and continuity
- Participate in interdisciplinary team meetings to improve quality of care and resident outcomes
Qualifications
- Licensed Social Worker (LMSW, LCSW, or state equivalent) or eligible for licensure in Maine
- Bachelor’s degree in Social Work (BSW) required; MSW preferred
- Experience in long-term care, behavioral health, or community case management is a plus
- Strong clinical assessment, documentation, and case management skills
- Comfortable working as part of an interdisciplinary team and advocating for clients
- Ability to pass required background checks and meet facility onboarding requirements
Education
- BSW (Bachelor of Social Work) required; MSW (Master of Social Work) preferred
Knowledge & Skills
- Knowledge of case management, psychosocial assessment, and care planning
- Skilled in crisis intervention and de-escalation techniques
- Excellent verbal and written communication for effective documentation and team coordination
- Familiarity with community resources, discharge planning, and referral processes
- Good organizational skills and the ability to manage a varied caseload
