Roles & Responsibilities

• Assess speech, language, cognitive-communication and swallowing function for residents
• Develop individualized treatment plans that focus on meaningful, measurable outcomes
• Implement evidence-based therapy to improve communication and swallowing safety
• Document progress and maintain timely, thorough clinical notes in the facility EHR
• Collaborate with nursing, PT/OT and caregivers to coordinate resident-centered care
• Educate staff and families on carry-over strategies and safe swallowing techniques
• Participate in care planning and interdisciplinary meetings to support quality outcomes

Qualifications

• Licensed or eligible for licensure as a Speech-Language Pathologist in Texas
• Experience providing SLP services in a skilled nursing/long-term care or rehab setting preferred
• Comfortable managing dysphagia evaluations and treatment
• Strong communication and documentation skills
• Ability to work independently while collaborating with an interdisciplinary team

Education

• Master's or clinical degree in Speech-Language Pathology (preferred/typical requirement for SLP roles)

Knowledge & Skills

• Skilled in speech, language, cognitive-communication and swallowing assessment
• Experience with individualized treatment planning and outcome measurement
• Proficient documentation and familiarity with clinical charting/EHR systems
• Ability to train and educate staff and family caregivers
• Strong clinical reasoning, time management and caseload prioritization
