Roles & Responsibilities
- Provide skilled therapeutic interventions to SNF residents to restore mobility and independence
- Perform assessments and implement individualized treatment plans with clear goals
- Monitor and document patient progress, updating care plans and communicating with the interdisciplinary team
- Educate residents and families on home programs and strategies to maintain gains
- Collaborate with nurses, therapists, and other staff to ensure safe and effective care
- Maintain accurate and timely documentation in accordance with facility policies and regulatory standards
Qualifications
- Current PTA credential or completion of an accredited PTA program (state licensure or eligibility as required)
- New grads welcome — years of experience: any, including New Grad
- Strong clinical reasoning and hands-on treatment skills appropriate for a SNF setting
- Reliable communication skills and the ability to work well within an interdisciplinary team
- Basic life support (BLS) certification as required by facility (obtainable if needed)
Knowledge & Skills
- Skilled in therapeutic exercise, gait training, transfers and mobility training
- Familiarity with SNF workflows and documentation expectations
- Patient-centered care approach with strong safety awareness
- Comfortable with clear documentation and electronic records
- Good time management and organizational skills for efficient caseload handling
