Roles & Responsibilities
- Evaluate and develop individualized treatment plans to help patients regain function
- Provide skilled therapeutic interventions and progress patients toward measurable goals
- Document patient care clearly, timely, and in accordance with facility standards
- Communicate regularly with the interdisciplinary team and families to coordinate care
- Educate patients and caregivers on home exercise programs and safe mobility strategies
- Contribute to a culture of safety and continuous improvement by following best practices
Qualifications
- Licensed Physical Therapist or eligible to obtain state licensure
- Active BLS/CPR certification or willing to obtain prior to start
- Comfortable working in a skilled nursing/rehabilitation setting
- Strong clinical reasoning, documentation skills, and patient-centered focus
- Professional, reliable, and ready for a 13-week travel contract assignment
Education
- Graduate of an accredited Physical Therapy program (DPT preferred)
Knowledge & Skills
- Skilled in assessment and treatment planning for post-acute populations
- Proficient in mobility training, therapeutic exercise, and transfer techniques
- Familiar with medical charting and documentation standards
- Excellent communication and teamwork skills with patients, families, and staff
- Ability to prioritize caseloads and adapt to changing clinical needs
