Roles & Responsibilities

• Provide compassionate bedside care to residents, prioritizing safety and dignity
• Administer medications and treatments accurately and document per facility policy
• Perform regular assessments and monitor changes in patient condition
• Collaborate with nurses, therapists, and care partners to create and follow care plans
• Communicate clearly with residents and families, offering education and support
• Maintain accurate electronic and paper records in a timely manner
• Follow infection control practices and facility safety procedures consistently

Qualifications

• Active Licensed Practical Nurse (LPN) or T-LPN credential
• Current clinical competence in long-term care or similar settings preferred
• Ability to pass facility-required background checks and health screenings
• Strong teamwork, communication skills, and a patient-centered approach

Education

• Graduate of an accredited practical nursing program; high school diploma or equivalent

Knowledge & Skills

• Skilled in medication administration and wound care basics
• Proficient in patient assessment and monitoring
• Familiarity with charting and documentation best practices
• Knowledge of infection control and resident safety procedures
• Good communication, organization, and time-management skills
