Candidate Pay Rate: $50 per hour

Roles & Responsibilities
- Evaluate patients and develop individualized, evidence-based physical therapy plans
- Deliver hands-on and therapeutic exercise interventions to improve mobility and function
- Document progress clearly and communicate goals and outcomes with patients and the care team
- Collaborate with nurses, OTs, SLPs, and case managers to coordinate patient-centered care
- Educate patients and caregivers on home programs, safety, and injury prevention
- Monitor and modify treatment plans based on patient response and functional gains
Qualifications
- Current state Physical Therapist license or ability to obtain Wisconsin license prior to start
- Active basic life support (BLS) certification or ability to obtain
- Strong clinical assessment and manual therapy skills in adult rehabilitation
- Excellent communication skills and comfort working in an interdisciplinary setting
- Reliable, professional, and committed to delivering high-quality patient care
Education
- Graduation from an accredited Physical Therapy program (DPT preferred)
Knowledge & Skills
- Skilled in musculoskeletal and functional mobility assessment
- Experience with therapeutic exercise prescription and gait training
- Familiarity with documentation systems and regulatory compliance
- Ability to prioritize caseload and deliver efficient, safe care
- Patient education and discharge planning expertise
