Roles & Responsibilities

• Develop and coordinate individualized post-acute discharge plans for patients with complex psychosocial needs
• Screen, assess, and connect patients and families to community resources addressing social drivers of health
• Provide crisis intervention and supportive counseling to patients and families during acute episodes
• Collaborate with Case Manager RN and interdisciplinary teams to ensure seamless transitions of care
• Conduct financial assessments and assist patients with eligibility determination for social programs and special funds
• Arrange in-home care, equipment, and patient transportation as part of safe discharge planning
• Refer patients to Behavioral Health and other outpatient services when formal assessment or follow-up is needed
• Educate patients and families on food access, housing resources, and community supports to improve outcomes

Qualifications

• California Licensed Clinical Social Worker (LCSW) required
• Bachelor’s degree in Social Work required; Master’s in Social Work preferred
• Three to five years of hospital social work experience preferred
• Demonstrated experience in discharge planning, crisis intervention, and care coordination
• Strong communication skills and the ability to work collaboratively with clinical teams
• Compassionate, resourceful, and able to manage complex psychosocial cases

Education

• Bachelors in Social Work (required)
• Masters in Social Work (preferred)

Knowledge & Skills

• Discharge planning and care coordination expertise
• Psychosocial assessment and crisis intervention skills
• Familiarity with community resource referral and social determinants of health
• Experience with patient transportation coordination and arranging in-home care
• Financial assessment and eligibility determination for social programs
• Ability to educate and support patients and families through transitions of care
