Roles & Responsibilities
- Provide compassionate bedside care and monitor resident health to ensure safety and comfort
- Administer medications and treatments accurately, following physician orders and facility protocols
- Perform routine assessments, document findings clearly, and communicate changes to the care team
- Collaborate with RNs, CNAs, therapists, and family members to deliver individualized care plans
- Support restorative and rehabilitative goals to help residents maintain independence
- Assist with admissions, discharges, and transfers while maintaining strong infection control practices
- Advocate for residents’ needs and contribute to a positive, person-centered care environment
Qualifications
- Active LPN license appropriate for practice (Massachusetts license required or eligible)
- Graduate of an accredited LPN/LVN program
- Current BLS/CPR certification (or willingness to obtain upon hire)
- Comfortable with medication administration, wound care basics, and documentation
- Successful background check and any required immunizations or screenings
- Team player with strong communication skills and a patient-first attitude
Education
- Completed accredited LPN/LVN program
- High school diploma or GED
Knowledge & Skills
- Skilled in vital signs monitoring, medication administration, and basic wound care
- Solid charting and documentation habits with attention to detail
- Ability to prioritize tasks in a fast-paced long-term care environment
- Strong interpersonal skills for working with residents, families, and multidisciplinary teams
- Familiarity with infection control and resident safety practices
