Roles & Responsibilities

• Provide direct patient care with compassion and clinical competence, including medication administration and monitoring
• Perform routine assessments and document patient status accurately to support the care team
• Assist with activities of daily living and promote patient comfort and dignity
• Collaborate with nurses, therapists, and interdisciplinary staff to implement individualized care plans
• Communicate clearly with patients and families to educate and involve them in care decisions
• Maintain infection control and safety practices to protect patients and staff
• Support rehabilitation goals and assist with restorative nursing interventions

Qualifications

• Current LPN license in Massachusetts or compact/state equivalent, required
• Completion of an accredited practical nursing program
• Current BLS/CPR certification, required
• Comfortable working 12-hour day shifts and adapting to a long-term care/rehab setting
• Strong communication skills and a team-oriented attitude
• Previous experience in long-term care or rehab settings preferred but not required

Education

• Graduate of an accredited practical nursing program
• LPN diploma or certificate required

Knowledge & Skills

• Medication administration and accurate charting
• Basic wound care and dressing change familiarity
• Skilled at patient assessments and recognizing changes in condition
• Good clinical judgment and prioritization in a busy nursing environment
• Strong interpersonal skills for patient, family, and team communication
• Time management and ability to handle multiple patient needs with calm
