Roles & Responsibilities
- Provide direct patient care with compassion and clinical competence, including medication administration and monitoring
- Perform routine assessments and document patient status accurately to support the care team
- Assist with activities of daily living and promote patient comfort and dignity
- Collaborate with nurses, therapists, and interdisciplinary staff to implement individualized care plans
- Communicate clearly with patients and families to educate and involve them in care decisions
- Maintain infection control and safety practices to protect patients and staff
- Support rehabilitation goals and assist with restorative nursing interventions
Qualifications
- Current LPN license in Massachusetts or compact/state equivalent, required
- Completion of an accredited practical nursing program
- Current BLS/CPR certification, required
- Comfortable working 12-hour day shifts and adapting to a long-term care/rehab setting
- Strong communication skills and a team-oriented attitude
- Previous experience in long-term care or rehab settings preferred but not required
Education
- Graduate of an accredited practical nursing program
- LPN diploma or certificate required
Knowledge & Skills
- Medication administration and accurate charting
- Basic wound care and dressing change familiarity
- Skilled at patient assessments and recognizing changes in condition
- Good clinical judgment and prioritization in a busy nursing environment
- Strong interpersonal skills for patient, family, and team communication
- Time management and ability to handle multiple patient needs with calm
