Roles & Responsibilities
- Provide compassionate direct patient care including ADLs and basic nursing tasks
- Administer medications and treatments safely and accurately according to plan
- Monitor patient status and report changes promptly to the interdisciplinary team
- Coordinate with nurses, therapists, and providers to ensure seamless care transitions
- Document assessments, interventions, and patient progress clearly in the chart
- Educate patients and families on care plans, medications, and safety at discharge
- Support a positive, team-focused environment that prioritizes patient dignity
Qualifications
- Active Licensed Practical Nurse (LPN) license — Massachusetts license preferred
- Current BLS/CPR certification (or willingness to obtain upon hire)
- Successful completion of background check, drug screen, and required health screenings
- Comfortable with electronic health record documentation
- Previous experience in long-term care, rehab, or skilled nursing settings preferred
Education
- LPN diploma or certificate from an accredited program
- High school diploma or GED required
Knowledge & Skills
- Strong clinical assessment and basic nursing skills
- Medication administration and safe documentation practices
- Wound care basics and pressure injury prevention
- Infection control principles and patient safety focus
- Clear communication skills with patients, families, and team members
- Time management and ability to prioritize in a busy care environment
