Roles & Responsibilities

• Provide compassionate direct patient care including ADLs and basic nursing tasks
• Administer medications and treatments safely and accurately according to plan
• Monitor patient status and report changes promptly to the interdisciplinary team
• Coordinate with nurses, therapists, and providers to ensure seamless care transitions
• Document assessments, interventions, and patient progress clearly in the chart
• Educate patients and families on care plans, medications, and safety at discharge
• Support a positive, team-focused environment that prioritizes patient dignity

Qualifications

• Active Licensed Practical Nurse (LPN) license — Massachusetts license preferred
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Successful completion of background check, drug screen, and required health screenings
• Comfortable with electronic health record documentation
• Previous experience in long-term care, rehab, or skilled nursing settings preferred

Education

• LPN diploma or certificate from an accredited program
• High school diploma or GED required

Knowledge & Skills

• Strong clinical assessment and basic nursing skills
• Medication administration and safe documentation practices
• Wound care basics and pressure injury prevention
• Infection control principles and patient safety focus
• Clear communication skills with patients, families, and team members
• Time management and ability to prioritize in a busy care environment
