Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients
• Administer medications and treatments accurately and safely
• Monitor and document vital signs, patient status, and care interventions
• Assist residents with activities of daily living and mobility needs
• Collaborate with nurses, therapists, and care team members to coordinate care
• Communicate clearly with families and the interdisciplinary team about patient needs
• Follow infection control practices and maintain a safe care environment
• Contribute to care plans and report changes in condition promptly

Qualifications

• Active Licensed Practical Nurse (LPN) license — required (Massachusetts preferred)
• Current BLS/CPR certification (required or obtained upon hire)
• Prior experience in long-term care, rehab, or skilled nursing is a plus
• Ability to pass background check and meet facility health requirements
• Strong communication skills and a team-oriented attitude

Education

• Diploma or certificate in Practical Nursing (or equivalent)
• Completion of an accredited LPN program and state licensure

Knowledge & Skills

• Medication administration and accurate documentation
• Basic patient assessment and vital sign monitoring
• Knowledge of infection prevention and wound care basics
• Care planning, reporting changes, and following care protocols
• Comfortable assisting with ADLs and supportive resident care
• Professional, compassionate communication with patients and families
