Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents and patients
- Administer medications and treatments accurately and safely
- Monitor and document vital signs, patient status, and care interventions
- Assist residents with activities of daily living and mobility needs
- Collaborate with nurses, therapists, and care team members to coordinate care
- Communicate clearly with families and the interdisciplinary team about patient needs
- Follow infection control practices and maintain a safe care environment
- Contribute to care plans and report changes in condition promptly
Qualifications
- Active Licensed Practical Nurse (LPN) license — required (Massachusetts preferred)
- Current BLS/CPR certification (required or obtained upon hire)
- Prior experience in long-term care, rehab, or skilled nursing is a plus
- Ability to pass background check and meet facility health requirements
- Strong communication skills and a team-oriented attitude
Education
- Diploma or certificate in Practical Nursing (or equivalent)
- Completion of an accredited LPN program and state licensure
Knowledge & Skills
- Medication administration and accurate documentation
- Basic patient assessment and vital sign monitoring
- Knowledge of infection prevention and wound care basics
- Care planning, reporting changes, and following care protocols
- Comfortable assisting with ADLs and supportive resident care
- Professional, compassionate communication with patients and families
