Roles & Responsibilities
- Provide compassionate direct nursing care to residents in a rehabilitation and long-term care setting
- Administer medications and treatments safely and document per facility policy
- Perform routine patient assessments and report changes to the care team
- Assist with activities of daily living and promote resident independence
- Maintain accurate, timely charting in the resident record
- Collaborate with nurses, therapists, and support staff to deliver coordinated care
- Support infection control practices and maintain a safe environment
Qualifications
- Current Licensed Practical Nurse (LPN) license (Massachusetts preferred)
- BLS/CPR certification required or must obtain upon hire
- Completion of an accredited LPN program
- Prior experience in long-term care or rehab settings preferred but not required
- Strong communication skills and a team-oriented mindset
- Ability to pass required health screenings and background checks
Education
- LPN diploma or certificate from an accredited program
- High school diploma or GED
Knowledge & Skills
- Medication administration and basic pharmacology knowledge
- Resident assessment and documentation best practices
- Wound care basics and restorative nursing principles
- Familiarity with electronic charting/EHR systems
- Infection prevention and safety procedures
- Effective communication with residents, families, and team members
