Roles & Responsibilities

• Evaluate patient functional abilities and design individualized occupational therapy plans that make daily life more manageable and meaningful
• Deliver evidence-based therapeutic interventions to improve independence, safety and participation in daily activities
• Collaborate with an interdisciplinary team to coordinate care and ensure continuity across services
• Provide clear, timely documentation and progress notes that support patient outcomes and regulatory requirements
• Educate patients and caregivers on home programs, adaptive strategies and assistive devices to maximize long-term success
• Contribute to a positive, patient-centered environment that emphasizes dignity and measurable progress

Qualifications

• Current and unrestricted Occupational Therapist license in Vermont or eligibility to obtain one
• NBCOT certification preferred or active credentials as required by facility
• Current CPR/BLS certification or willingness to obtain upon hire
• Strong clinical assessment and treatment planning skills with comfort using therapeutic modalities and adaptive equipment
• Excellent communication and teamwork skills to work effectively with patients, families and staff

Knowledge & Skills

• Skilled in functional assessments, ADL/IADL training and adaptive equipment recommendations
• Knowledge of documentation standards and third-party documentation requirements
• Ability to create measurable, patient-centered goals and demonstrate progress
• Familiarity with discharge planning and community resource coordination
• Comfortable working within interdisciplinary teams and mentoring support staff when needed
