Roles & Responsibilities
- Evaluate patient functional abilities and design individualized occupational therapy plans that make daily life more manageable and meaningful
- Deliver evidence-based therapeutic interventions to improve independence, safety and participation in daily activities
- Collaborate with an interdisciplinary team to coordinate care and ensure continuity across services
- Provide clear, timely documentation and progress notes that support patient outcomes and regulatory requirements
- Educate patients and caregivers on home programs, adaptive strategies and assistive devices to maximize long-term success
- Contribute to a positive, patient-centered environment that emphasizes dignity and measurable progress
Qualifications
- Current and unrestricted Occupational Therapist license in Vermont or eligibility to obtain one
- NBCOT certification preferred or active credentials as required by facility
- Current CPR/BLS certification or willingness to obtain upon hire
- Strong clinical assessment and treatment planning skills with comfort using therapeutic modalities and adaptive equipment
- Excellent communication and teamwork skills to work effectively with patients, families and staff
Knowledge & Skills
- Skilled in functional assessments, ADL/IADL training and adaptive equipment recommendations
- Knowledge of documentation standards and third-party documentation requirements
- Ability to create measurable, patient-centered goals and demonstrate progress
- Familiarity with discharge planning and community resource coordination
- Comfortable working within interdisciplinary teams and mentoring support staff when needed
