Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients
• Administer medications and treatments safely and accurately
• Perform focused assessments and promptly communicate changes to the care team
• Document care thoroughly and maintain accurate electronic medical records
• Assist with activities of daily living and support resident comfort and dignity
• Collaborate with RNs, therapists, and facility staff to deliver coordinated care
• Participate in care planning and quality improvement efforts to enhance outcomes

Qualifications

• Current LPN license or LPN/LVN credential in New Mexico or eligibility for state licensure
• Current BLS/CPR certification required
• Graduate of an accredited practical nursing program
• Previous experience as an LPN preferred, especially in long term care or rehab settings
• Ability to pass background check and meet facility immunization requirements
• Strong communication skills and a team-oriented attitude

Education

• Completion of an accredited Practical Nursing (LPN/LVN) program

Knowledge & Skills

• Medication administration and basic nursing treatments
• Resident assessment, vital signs, and documentation best practices
• Infection control and wound care fundamentals
• Familiarity with electronic medical record charting
• Strong time management, prioritization, and teamwork skills
• Empathy, patient-centered care, and clear communication with families
