Roles & Responsibilities

• Provide compassionate hands-on care to residents, assisting with activities of daily living
• Support safe transfers and mobility while using proper body mechanics
• Take and record vital signs, noting and reporting any changes in condition
• Document care accurately and promptly in the patient chart or electronic record
• Follow individualized care plans and support therapy or activity programs
• Communicate clearly and kindly with residents, families and the interdisciplinary team
• Maintain a clean, safe environment and adhere to infection control policies

Qualifications

• Active GNA certification (Geriatric Nursing Assistant) or equivalent required
• Current CPR/BLS certification preferred or obtain within hiring timeframe
• High school diploma or GED preferred
• Previous experience in long-term care, skilled nursing or geriatric settings is a plus
• Ability to pass background check, drug screen and meet facility onboarding requirements
• Strong interpersonal skills and a genuine desire to work with older adults

Education

• High school diploma or GED required or equivalent
• GNA certification required

Knowledge & Skills

• Comfortable with resident care tasks including bathing, dressing and toileting assistance
• Skilled in measuring and recording vital signs and observing clinical changes
• Knowledge of basic infection prevention and proper PPE use
• Good documentation habits and basic computer/EHR familiarity
• Ability to safely assist with mobility and transfers using proper techniques
• Strong communication and teamwork skills with a resident-centered focus
