Roles & Responsibilities

• Provide compassionate hands-on care to geriatric patients, assisting with daily living activities
• Assist the rehab team with mobility support and safe patient transfers
• Monitor and record vital signs and patient status with attention to detail
• Document care provided and report changes promptly to nursing staff
• Support a clean, safe, and comfortable environment for patients and families
• Communicate respectfully and clearly with patients, families, and interdisciplinary team members
• Participate in patient care planning and follow-through to help achieve rehab goals

Qualifications

• Active GNA certification (required)
• Experience in geriatric, long-term care, or rehabilitation settings preferred
• Ability to pass background checks and health screenings required by the facility
• Comfortable performing physical aspects of patient care and safe transfer techniques
• Strong communication skills and a team-first attitude

Education

• High school diploma or GED preferred
• GNA certification (required)

Knowledge & Skills

• Understanding of ADLs (activities of daily living) and geriatric care needs
• Basic vital sign measurement and documentation skills
• Familiarity with infection control and patient safety practices
• Ability to follow care plans and verbal/written instructions accurately
• Compassion, patience, and excellent interpersonal skills
• Time management and the ability to prioritize tasks in a busy rehab setting
