Roles & Responsibilities

• Provide compassionate daily care to residents, helping with bathing, grooming and ADLs
• Assist with safe transfers and mobility support to promote independence
• Monitor vital signs and observe changes in condition, reporting concerns promptly
• Document care accurately and clearly in the resident record
• Support restorative and rehab activities under the direction of clinical staff
• Maintain a clean, safe environment and follow infection control practices
• Collaborate with nurses, therapists and family members to deliver person-centered care

Qualifications

• Current GNA certification or equivalent required
• Valid state-specific nursing assistant certification or ability to obtain one
• Current CPR/BLS certification preferred or willingness to obtain
• Comfort working with older adults in a rehab/long-term care setting
• Strong communication skills and a team-first attitude
• Background checks and health screening clearance as required by facility

Education

• High school diploma or equivalent
• GNA certification (required)

Knowledge & Skills

• Understanding of geriatric care principles and common age-related conditions
• Basic clinical skills: taking vitals, assisting with ADLs, safe patient handling
• Familiarity with infection control and safety standards
• Good documentation and observation skills
• Compassionate bedside manner and strong interpersonal skills
• Ability to follow care plans and work within a multidisciplinary team
