Roles & Responsibilities

• Administer medications and treatments accurately and safely according to care plans
• Monitor patient status and report changes promptly to the care team
• Perform patient assessments and document findings in the medical record
• Provide compassionate daily care and support to residents at Hillcrest Center
• Collaborate with RNs, physicians, and interdisciplinary teams to deliver great outcomes
• Educate patients and families about care plans, medications, and follow-up
• Maintain infection control standards and promote a safe care environment

Qualifications

• Active LPN license in Pennsylvania or Active RN license in Pennsylvania, as applicable
• Basic Life Support (BLS/CPR) certification required
• Previous experience in long-term care, skilled nursing, or similar healthcare settings preferred
• Strong clinical assessment, medication administration, and documentation skills
• Excellent communication and teamwork skills with a patient-centered approach
• Ability to pass background checks and meet facility onboarding requirements

Education

• LPN: Graduate of an accredited Practical Nursing program and state licensure
• RN: Associate Degree in Nursing (ADN) or Nursing Diploma with active RN licensure

Knowledge & Skills

• Medication administration and accurate charting
• Vital signs and basic clinical assessments
• Care plan implementation and patient-centered care
• Infection prevention and safety protocols
• Clear verbal and written communication with patients and teammates
• Comfort working with electronic medical records and basic computer skills
