Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents, promoting safety and dignity
• Administer medications and treatments accurately and on time
• Assess, monitor and document patient conditions, reporting changes promptly
• Collaborate with the interdisciplinary team to coordinate individualized care plans
• Perform wound care, dressing changes and basic procedures as needed
• Support resident and family education about care, medications and follow-up
• Maintain accurate and timely electronic medical records and shift reports

Qualifications

• Active LPN license (must be eligible to practice in the assignment state)
• Current BLS/CPR certification or ability to obtain prior to start
• Demonstrated experience in long-term care or similar clinical setting preferred
• Comfortable working evenings and weekends as required by the schedule
• Strong communication skills and ability to work effectively on a travel assignment

Education

• Graduation from an accredited Practical Nursing (LPN) program and active LPN licensure

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and infection control procedures
• Vital signs assessment and clinical observation skills
• Familiarity with electronic medical records (EMR) and charting
• Ability to prioritize, multitask and provide compassionate resident-centered care
• Team collaboration and clear communication with clinical staff and families
