Roles & Responsibilities
- Provide compassionate, patient-centered bedside care to residents in a skilled nursing environment
- Administer medications and treatments accurately and document in the medical record
- Monitor patient condition and report changes promptly to the interdisciplinary team
- Assist with admissions, discharges, and care transitions to ensure continuity of care
- Collaborate with RNs, therapists, and support staff to develop and follow individualized care plans
- Educate patients and families on care, treatment plans, and follow-up needs
Qualifications
- Current Licensed Practical Nurse (LPN) license in New Mexico (required)
- Basic Life Support (BLS) certification or willingness to obtain prior to start
- Prior experience in long-term care, skilled nursing, or related clinical settings preferred
- Clear background check and ability to meet facility onboarding requirements
- Comfortable working as a travel LPN and adapting to new team environments
Education
- Vocational/Practical Nursing diploma or equivalent; successful completion of LPN program
Knowledge & Skills
- Strong medication administration and charting skills
- Solid patient assessment and documentation abilities
- Good communication and teamwork with interdisciplinary staff and families
- Ability to manage time and prioritize tasks during busy shifts
- Familiarity with infection control standards and resident safety practices
