Roles & Responsibilities

• Provide compassionate, patient-centered bedside care to residents in a skilled nursing environment
• Administer medications and treatments accurately and document in the medical record
• Monitor patient condition and report changes promptly to the interdisciplinary team
• Assist with admissions, discharges, and care transitions to ensure continuity of care
• Collaborate with RNs, therapists, and support staff to develop and follow individualized care plans
• Educate patients and families on care, treatment plans, and follow-up needs

Qualifications

• Current Licensed Practical Nurse (LPN) license in New Mexico (required)
• Basic Life Support (BLS) certification or willingness to obtain prior to start
• Prior experience in long-term care, skilled nursing, or related clinical settings preferred
• Clear background check and ability to meet facility onboarding requirements
• Comfortable working as a travel LPN and adapting to new team environments

Education

• Vocational/Practical Nursing diploma or equivalent; successful completion of LPN program

Knowledge & Skills

• Strong medication administration and charting skills
• Solid patient assessment and documentation abilities
• Good communication and teamwork with interdisciplinary staff and families
• Ability to manage time and prioritize tasks during busy shifts
• Familiarity with infection control standards and resident safety practices
